If you have not acquired another citizenship before the deadline, you will not be regarded as released, and will continue to be a Norwegian citizen.
There is no specific application form. It is sufficient that you write a letter to the Norwegian Directorate of Immigration. If you live abroad, you need to deliver the letter to the nearest Norwegian foreign service mission (embassy or general consulate). If you live in Norway, you need to deliver the letter to your local police office.
You need to document which of the abovementioned conditions are met. If you live in Norway, you need to give information of why you feel it will be unreasonable to deny you to be released from Norwegian citizenship.
